Job Summary:
Physical Therapist (Home Health) in a home health setting within the designated geographic areas. This position involves providing patient evaluations, therapy planning, and delivering skilled physical therapy services to patients in their homes. The role requires flexibility to travel within the assigned areas and may include weekend coverage as needed.

Job Details:

• Schedule: Days shift, 9 hours per day, minimum 36 hours per week
• Required Qualifications: Current California Physical Therapist License, BLS certification, valid Drivers License, auto insurance and registration, TB test (annual), MMR, Varicella, HepB, TDaP (valid for 10 years), annual Flu vaccine, Drug screen, and Physical
• Position Description: Assignments will be in designated counties covering areas #4 and #5, with potential for additional travel outside these zones. The role may require weekend work and traveling outside the service area to maintain caseload. Payment is made on a per visit basis, covering all travel and documentation time.
• Start Date: 10/12/2026
• End Date: 01/11/2027
• Additional Information:
  • The position involves clinical documentation including OASIS assessments, routine evaluations, discharges, and recertifications.
  • Traveler must complete at least 27 points weekly or be willing to travel outside the service area or work weekends if guarantee is preferred.
  • All requested time off (RTO) must be disclosed at submission; post-approval RTO requests are not permissible.
  • Agencys contract must match the official confirmation terms, including start and end dates.
