Centra Healthcare Allied is seeking a travel Physical Therapist for a travel assignment in Chisholm, Minnesota. This contract is for 13 weeks, with 40 hours per week and 8-hour days in a SNF/LTC setting.

Ideal candidates have graduated from an accredited program, hold a current PT license or are in process, CPR certification, and can develop and execute care plans while providing skilled PT services per physician orders, collaborating with the rehab team.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or in process of licensure.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions per physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
