Job Summary

Physical Therapist Home Health (PT HH) position available in the healthcare sector, located in a designated territory of up to a 60-mile radius from the hub. This role involves providing home health services to geriatric patients, with a focus on timely documentation, effective communication, and flexible territory coverage. The position offers a schedule of dayshift hours, with a commitment of 40 hours per week and an initial start date of October 19, 2026.

Job Details

Schedule: Dayshift hours, 8 hours per day, minimum of 40 hours per week.

Required Qualifications

Bachelor's or Master's degree in Physical Therapy, valid and active state licensure in hand, at least 1 year of relevant experience, proficiency in Homecare Homebase (HCHB) electronic charting system, and current BLS certification.

Certifications: BLS certification required.

Experience: Minimum of 1 year of relevant experience; OASIS experience required for staff completing Start of Care in Home Health; no first-time travelers accepted.

Additional Information

On-call rotation required, to be discussed during interview.

Travelers should be flexible with territory coverage and possess own transportation.

Orientation scheduled on Monday.

All travelers must have an active drivers license and submit requests for time off at the time of application.

Travel pairs are accepted; each traveler must have own territory and transportation.

Modules are billable and to be completed during orientation.

The assignment duration is from 10/19/2026 to 01/18/2027.

Pay package is based on 8 hour shifts and 40.0 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
