Job summary

Wellspring Nurse Source seeks a travel Physical Therapist to join our Milwaukee, Wisconsin team. The role focuses on patient evaluation, treatment planning, and delivery of direct therapy to restore function and mobility.

You will work on a 52-week assignment with 40 hours per week, days shift, and supervision of PTAs in accordance with state regulations. Our team supports clinicians with resources and a strong professional network.

Qualifications

• Graduate of an APTA-accredited Physical Therapy program (DPT/MSPT/BSPT).
• Active state Physical Therapist license or ability to obtain licensure.
• Strong communication, documentation, and clinical skills.
• Flexibility to adapt to varying facilities, schedules, and patient needs.

Responsibilities

• Screen, evaluate, and treat patients as appropriate.
• Develop and update treatment and discharge plans of care.
• Document services accurately and in a timely manner.
• Supervise PTAs and Rehabilitation Technicians in accordance with practice acts.
• Collaborate with interdisciplinary care teams and families.
• Maintain compliance with safety, infection control, and regulatory standards.
