Position Description: Outpatient Physical Therapist responsible for evaluating assigned patients, developing, implementing, and modifying treatment plans aligned with patient needs, goals, and physician directions. Responsible for delegation and direction of all discipline-specific therapy services for designated patients, ensuring functional treatment programs to facilitate effective return to community activities.
Required Qualifications: Degree in Physical Therapy from an accredited school; at least 1 year of outpatient clinical experience; CPR certification.
Schedule: Days shift, 8 hours per day, with a minimum of 40 hours per week.
Additional Information: Completes comprehensive patient evaluations, documents medical records accurately and timely, ensures care is provided based on physician orders and insurance approval, and participates in student training and professional development activities.
