Job Details

Position Description: Outpatient Physical Therapist responsible for evaluating assigned patients, developing, implementing, and modifying treatment plans aligned with patient needs, goals, and physician directions. Responsible for delegation and direction of all discipline‑specific therapy services for designated patients, ensuring functional treatment programs to facilitate effective return to community activities.
Required Qualifications: Degree in Physical Therapy from an accredited school; at least 1 year of outpatient clinical experience; CPR certification.
Schedule: Days shift, 8 hours per day, with a minimum of 40 hours per week.
Additional Information: Completes comprehensive patient evaluations, documents medical records accurately and timely, ensures care is provided based on physician orders and insurance approval, and participates in student training and professional development activities. Maintains licensure and adheres to organizational performance improvement initiatives.
