Job summary

Jackson Therapy Partners is seeking a travel Physical Therapist for a SNF assignment in Laconia, New Hampshire. The position offers 40 hours per week across days, with a 13‑week duration and an ASAP start. You’ll travel to provide patient rehabilitation and support in diverse SNF settings.

The ideal candidate holds a PT degree, an active state license, and an optional BLS certification; SNF experience is valued and care coordination is a focus in this role.

Qualifications

• Required degree in Physical Therapy (BPT, MSPT or DPT) from an approved program.
• Active state license to practice as a Physical Therapist.
• BLS certification may be required.
• Experience in a Skilled Nursing Facility (SNF).

Responsibilities

• Float to sister location as needed.
• Provide Physical Therapy evaluation and treatment in SNF setting.
• Collaborate with nursing staff to optimize patient mobility and function.
