Centra Healthcare Allied is seeking a travel Physical Therapist for a 13-week assignment in Pueblo, Colorado. The role involves providing skilled PT services in a SNF/LTC setting, with a 40-hour workweek and dayshift.

Start date is ASAP, and the contract emphasizes accountability to the organization’s mission and goals. The candidate should be a graduate of an accredited PT program, hold a current state license (or be in the licensure process) and maintain CPR certification, with collaboration

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist or in process.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services per physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.

Responsibilities

• Join a Skilled Nursing Facility / LTC team on a contract assignment.
• Provide PT services 40 hours per week as scheduled.
• Adhere to organization vision, values, and goals.
• Ensure care aligns with physician orders and patient goals.
