Healthforce is seeking a Physical Therapist for a 13-week travel assignment in a skilled nursing facility in Laconia, NH. Start date is 10/05/2026 with guaranteed 35 hours per week and 5x8 shifts. Travel and onboarding support are provided, with competitive weekly pay and credentialing assistance.

Qualifications

• Active physical therapy license in good standing; NH licensure or eligibility may be required per regulatory standards.
• Strong communication and documentation skills.
• BLS certification or facility policy required.
• Experience in skilled nursing facility or geriatric rehab settings preferred.
• Ability to work in a multidisciplinary team in a fast-paced care environment.
• Willingness to travel and commit to a 13-week assignment.

Responsibilities

• Perform initial PT evaluations and develop individualized treatment plans for residents in the skilled nursing setting.
• Deliver evidence-based physical therapy interventions to improve mobility, strength, balance, and functional independence.
• Monitor patient progress, document outcomes, and adjust treatment plans with the interdisciplinary team.
• Educate patients and families on home programs, safety, and fall-prevention strategies; ensure patient safety during therapy sessions.
• Maintain accurate, timely documentation in the medical record and adhere to facility policies and regulatory requirements.
