• Must have Doctorate in Physical Therapy or Bachelor's in Physical Therapy with NPTE
• Graduated from an accredited school
• Minimum one year of experience
• Current state PT licensure or in process
• Current CPR certification

• Provides skilled physical therapy services per physician orders
• Assesses patient needs and adjusts care to goals
• Collaborates with all disciplines to plan and evaluate goals
