Job summary

Healthforce is seeking a Physical Therapist for a Skilled Nursing Facility in Cedar Grove, New Jersey. This travel - temporary assignment starts on 10/18/2026 for 12 weeks, with guaranteed hours of 35 per week and a scheduled shift from 08:00 to 16:30 (5x8 days).

Responsibilities include assessing residents’ needs, developing plans of care, implementing interventions, and documenting progress. Collaboration with nursing, OT, SLP, and physicians is required.

Qualifications

• Current, valid Physical Therapist license to practice in New Jersey or eligibility for NJ licensure.
• Ability to work in a skilled nursing facility environment and participate in a 12-week travel assignment.
• Strong documentation and communication skills; comfortable collaborating with an interdisciplinary team.
• Availability to start on or before 10/18/2026 and commit to the 12-week assignment in Cedar Grove, NJ.
• Demonstrated professionalism and reliability in clinical settings.

Responsibilities

• Assess residents’ physical therapy needs and develop individualized plans of care based on evaluation findings and goals.
• Implement therapeutic interventions, including therapeutic exercises, gait training, balance training, and functional mobility activities.
• Monitor patient progress and adjust treatment plans to achieve documented rehabilitation goals.
• Document treatments, progress, and outcomes in the medical record per facility policy and regulatory requirements.
• Collaborate with the interdisciplinary team (nursing, occupational therapy, speech therapy, physicians) to coordinate care and discharge planning.
• Ensure patient safety, proper documentation, and infection control during therapy sessions.
