Job summary

Elara Caring is seeking a Travel Physical Therapist to provide high-quality care in patients' homes across the travel region. You will evaluate functional needs, design plans of care, and coordinate with teams to achieve optimal outcomes.

The role emphasizes autonomy, 1:1 patient care, and adherence to HIPAA/privacy standards. Requirements include graduation from an APTA-approved PT program, 1+ year experience, licensure in travel states, and ability to travel 100% with a valid driver’s license

Qualifications

• Graduate of an APTA-approved Physical Therapy Program.
• 1+ year experience as a Physical Therapist or Physical Therapy Aide.
• Current, unrestricted licenses from the State Board of Physical Therapy Examiners for all states within travel region.

Responsibilities

• Provide direct patient care and evaluate functional needs and outcomes.
• Design and implement a care plan based on thorough assessment.
• Direct the assistance of support staff to achieve patient goals.
• Document evaluations, goals, plans, and progress.

As a Travel Physical Therapist, you'll contribute to our success in the following ways:

• Ensures that all activities performed align with the vision of Elara Caring's board of directors, executive team, and the leadership of the Home Health team.
• Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists. Reviews patients' functional needs and adjusts care plans to ensure optimal patient outcomes.
• Designs and implements a plan of care for patients based on a thorough assessment of potential development and incorporates participation by significant others whenever it seems appropriate.
• Conducts therapy practice within defined standards of care.
• Directs the assistance of support staff to achieve patients' functional goals.
• Communicates effectively with other professionals and support staff to achieve positive patient outcomes.
• Practices hospital and departmental policies and procedures, objectives, quality improvement program, safety, environmental, and infection control standards.
• Documents evaluations, treatment goals, and plans. Regularly updates documentation and maintains prescriptions and signatures.
• Enhances professional growth and development of self and others through participation in educational programs, in-service meetings, and workshops. Keep up to date on professional literature.
• Provides direction and leadership to staff and acts as a resource to promote physical therapy in the community.
• Promotes Elara Caring's philosophy, mission statement and administrative policies to ensure quality of care.
• Maintains patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule.
• Performs other duties/projects as assigned.

What is Required?

• Graduate of an American Physical Therapy Association (APTA) approved Physical Therapy Program
• 1+ year experience as a Physical Therapist or Physical Therapy Aide
• Current, unrestricted licenses from the State Board of Physical Therapy Examiners for all states assigned within travel region.
• Must have a dependable vehicle, valid drivers licence, and current auto insurance in accordance with the laws of the state.
• Full range of body motion including handling, lifting and transfer of patients
• Must be able to lift up to 100lbs
• Ability and willingness to travel 100%

You will report to the Clinical Team Manager.
