Active, unrestricted Pharmacist license

Bachelor or Doctor of Pharmacy

Minimum 2 years of pharmacist experience

Current BLS certification

IV admixture and immunization certification

Active, unrestricted Pharmacy Tech certification

Minimum 1 year of technician experience

Current BLS certification for Technicians

Education
