Qualifications

• Minimum 2 years of hospital experience.
• Experience with complex patients (trauma, critical care, pediatrics, cardiac surgery, ECMO).
• Experience with Dose Edge, Pyxis, Meditech, and Vigilanz.
• Advanced dosing skills: vancomycin AUC, aminoglycosides, renal dosing, warfarin, LMWH, DOACs.

Responsibilities

• Provide inpatient pharmaceutical care in a hospital unit.
• Monitor and optimize dosing for high-acuity patients.
• Utilize Dose Edge, Pyxis, Meditech, and Vigilanz for medication management.
• Collaborate with clinicians and nursing staff to ensure patient safety.
