Job summary

Centra Healthcare Allied is seeking a travel Physical Therapist for a 12-week contract in Castro Valley, California. This outpatient position offers 40 hours per week, days shift, and travel compensation.

Start date ASAP. Requirements include graduation from an accredited program, current PT license in California or eligibility, CPR certification, and ability to develop a plan of care and provide skilled PT interventions.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist or in the process of licensure.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions per physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals.

Responsibilities

• Develop and implement individualized treatment plans for each patient.
• Provide skilled PT interventions in line with physician orders.
• Collaborate with the care team to set goals and monitor progress.
• Document patient progress and update care plans accordingly.
