Job summary

Centra Healthcare Allied is seeking a travel Physical Therapist for a travel assignment in Glendale, Arizona. The role is outpatient and requires an accredited degree, licensure in good standing or in process, CPR, and the ability to develop and execute a care plan for each patient.

The position is a 13-week contract with a typical 40 hours per week schedule and an 8-hour shift. This contract emphasizes mobility and collaboration with a multidisciplinary team to deliver high-quality care.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist or in the process of licensure.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services in accordance with physician orders.
• Assess patient needs, plan for, evaluate and modify care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
