Responsibilities

• Assess, plan, evaluate and implement care for patients assigned physical therapy
• Gather pertinent data from chart and discuss with physician as necessary
• Independently assess all patient types as assigned
• Reassess progress and set measurable goals
• Identify equipment needs and coordinate with patient and family
