Job summary

Centra Healthcare Allied is seeking a travel Occupational Therapist for a contract assignment in Hazel Crest, Illinois. The role is a 13-week travel position, with 40 hours per week on a days shift, in a SNF/LTC setting.

Ideal candidates have graduated from an accredited OT program, hold or are pursuing state licensure, and possess CPR certification. The OT will provide services per physician orders and collaborate with a multidisciplinary team to meet patient goals.

Qualifications

• Must have graduated from an accredited school (OT program).
• Current state license as an Occupational Therapist or in the process of licensure.
• Current CPR certification.
• Provide skilled occupational therapy services per physician orders.
• Assess patient needs, plan, evaluate and modify care for OT interventions.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.
