Qualifications

Active OT therapy credentials and ability to work in SNF environment.
Willingness to commit to a 16-week travel assignment.

Responsibilities

Provide OT evaluation, treatment planning, and interventions for adult residents in SNF.
Implement and modify therapy programs to promote independence in ADLs and mobility.
Document progress, goals, and discharge planning in the EHR.
Collaborate with an interdisciplinary team to coordinate care.
Ensure safety during sessions and manage equipment and infection control.
Contribute to discharge planning and progression toward goals.
