Job summary

HealthForce Healthcare seeks an Occupational Therapist for a 15-week travel assignment in Lebanon, NH. You will perform OT evaluations, develop treatment plans, and collaborate with an interdisciplinary team to support discharge and community reintegration.

The role requires NH licensure or eligibility by endorsement, an OT degree (OTD or MS), and geriatric rehab experience, with a fixed 08:00–16:30 shift and 35 guaranteed hours per week.

Qualifications

• Active Occupational Therapist license in NH or eligibility by endorsement.
• Degree from an accredited OT program (OTD or Master’s).
• Ability to work the scheduled shift 08:00–16:30 and travel for 15 weeks.
• Experience in geriatric rehabilitation or Skilled Nursing Facility.
• Legally eligible to work in the United States and complete travel credentialing.

Responsibilities

• Perform comprehensive OT evaluations for SNF residents and set goals to improve ADLs, mobility, cognition, and safety.
• Plan and implement individualized treatment programs using therapies and adaptive equipment; track progress.
• Document assessments, plans, and progress in the chart adhering to privacy and regulations.
• Collaborate with an interdisciplinary team to support discharge planning and coordinated care.
• Educate residents and families on ADL strategies, safety, and community reintegration.
• Participate in transition or discharge planning to optimize continuity of care.
