Qualifications

• Active Washington occupational therapy license.
• Ability to provide OT services in a skilled nursing facility.

Responsibilities

• Conduct initial and ongoing evaluations of residents’ functional abilities and OT needs.
• Develop, implement, and adjust individualized treatment plans and measurable goals.
• Deliver direct OT services including activities of daily living training, ROM, motor planning, and adaptive equipment coaching.
• Monitor progress, document patient status, and communicate findings to the care team.
• Participate in interdisciplinary team meetings and contribute to discharge planning and transitions.
• Ensure patient safety and maintain compliant documentation and regulatory standards.
