Job summary

Centra Healthcare Allied is seeking a travel Occupational Therapist for a contract assignment in Modesto, California. This SNF/LTC setting offers a 13-week assignment with 40 hours per week and 8-hour shifts, starting ASAP.

The role requires graduation from an accredited OT program, active state licensure (or in process), and the ability to provide skilled OT services per physician orders while collaborating with a multidisciplinary team.

Qualifications

• Must have graduated from an accredited OT program.
• Current state license as an Occupational Therapist (OT) or in process of licensure.
• Provide skilled OT services per physician orders.
• Assess patient needs, plan for, evaluate and modify care to meet OT goals.
• Collaborate with all disciplines to plan and evaluate team goals.

Responsibilities

• Provide skilled OT services to patients in accordance with physician orders.
• Assess patient needs and develop, implement and modify OT intervention plans.
• Collaborate with multidisciplinary team to achieve patient goals.
