Job summary

Apex Home Health, Hospice & Home Care in Granger, IN is seeking a licensed Occupational Therapist to assess referred patients and provide rehabilitation services within interdisciplinary teams.

You will develop and implement individualized treatment plans, deliver skilled interventions, monitor progress, and modify care as needed in alignment with physician orders; CPR certified with at least one year of experience is preferred.

Qualifications

• Master's degree in Occupational Therapy or equivalent.
• Graduation from an accredited OT program.
• NBCOT exam passed.
• Current state licensure or active license application.
• Minimum 1 year professional OT experience.
• CPR certification required.

Role:
