Job summary

Occupational Therapist – General position in Seattle, WA with Healthforce Healthcare LLC. 13-week contract starting 11/30/2026, guaranteed 35 hours per week at $2,272–$2,338 weekly pay.

Shifts are 08:00–16:30, 5x8 days in a skilled nursing facility setting. Responsibilities include conducting assessments, developing individualized plans, delivering therapy, documenting progress, and collaborating with nursing, therapy colleagues, social work, and discharge planning teams.

Qualifications

• Licensed OT in WA or eligibility for WA licensure.
• Experience in skilled nursing facility settings.
• Strong assessment and documentation skills; effective communication.

Responsibilities

• Conduct initial and ongoing OT assessments for residents.
• Develop and implement individualized treatment plans.
• Deliver direct therapy sessions and monitor progress toward goals.
• Document evaluations, treatment plans, progress notes, and outcomes in compliance with regulations.
• Collaborate with an interdisciplinary team to optimize function and support discharge planning.
• Educate residents and families on goals, home exercise programs; ensure HIPAA compliance.

Responsibilities

• Conduct initial and ongoing occupational therapy assessments for residents
• Develop and implement individualized treatment plans targeting activities of daily living, mobility, upper-extremity function, and cognition
• Deliver direct therapy sessions and monitor progress toward functional goals
• Document evaluations, treatment plans, progress notes, and outcomes in compliance with regulatory and facility requirements
• Collaborate with an interdisciplinary team (nursing, therapy colleagues, social work) to optimize function and support discharge planning
• Educate residents and families on treatment goals, therapeutic activities, and home exercise programs; ensure safety and HIPAA compliance

Requirements

• Active Occupational Therapy license in Washington state or eligibility for WA licensure
• Ability to work effectively in a skilled nursing facility setting
• Strong assessment, documentation, and communication skills
• Availability to work the assigned shift and duration of the contract
