The role requires Vermont OT licensure (may be pending) and BLS certification, with prior travel experience. You will evaluate residents, develop treatment plans, document outcomes in the EMR, and collaborate with the care team.

Qualifications

Responsibilities

• Perform comprehensive OT evaluations for residents in a long-term care setting.

• Monitor progress, document outcomes, and adjust plans in collaboration with the interdisciplinary team.
• Provide patient and family education on therapeutic exercises, adaptive equipment, and safety strategies.
• Maintain accurate records in the EMR and participate in care planning meetings with nursing and medical staff.
• Attend bi-weekly orientations on Mondays and comply with site onboarding and policies.

Rates are tiered by mileage (direct line) for Travel assignments: 101 miles+ qualifies as Travel, 51–100 miles is Radius, 50 miles or less is Local. This assignment requires Vermont OT licensure (may be pending) and BLS certification.
