Qualifications

Responsibilities

• Evaluate and treat patients in short-term and long-term rehab settings.
• Develop individualized, evidence-based treatment plans.
• Deliver interventions to improve function and independence.
• Collaborate with interdisciplinary teams.
• Educate patients and caregivers throughout rehab.
• Track progress and adapt plans as needed.
• Support safe discharge planning.
