A great SNF / LTC is looking for an experienced or newly graduated Occupational Therapist / OT to join their growing team for this contract assignment.

The Occupational Therapist / OT is responsible for the assessment of referred patients that require rehabilitation services, including providing direct patient care to assess medical condition, functional capabilities, limitations, and potential for rehabilitation. The OT will establish and administer a treatment program with goals tailored to each patient’s capacity and tolerance under physician direction. The OT must set realistic and achievable goals, document and record patient condition, and educate patients and families in appropriate occupational therapy methods.

Requirements

• Must have graduated from an accredited school.
• Current state license as an Occupational Therapist / OT, or actively in the process of obtaining licensure.
• Current CPR certification.
• Provide skilled occupational therapy services/interventions in accordance with physician orders.
• Assess patient needs, plan, evaluate, and modify care to meet occupational therapy goals.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.
