Day Shift: Monday to Friday, 8:00 AM - 4:30 PM, with weekend rotation. Must work on Thanksgiving and New Year's.

Job Summary

The Occupational Therapist plays a key role as a member of the interdisciplinary team, responsible for developing, administering, and implementing therapeutic services aimed at helping patients regain their optimal level of function in daily activities including self-care, leisure, and work. This position reports to the Home Health and Hospice Manager.

Essential Job Functions

• Evaluate patients' functional status and implement the physician's plan of care.
• Conduct assessments and reassessments, making informed decisions based on objective tests and measurements.
• Communicate observations, facts, and treatment suggestions with team members to ensure coordinated care.
• Provide guidance and education to patients and coworkers as required.
• Demonstrate the ability to work independently and cooperatively within a team.
• Uphold and protect patient rights at all times.
• Exhibit empathy and compassion toward individuals experiencing grief, pain, or trauma.
• Document all communications, coordination, and treatment activities accurately and concisely.
• Adapt to emergency situations and remain vigilant to changes in patients' conditions.
• Support the organization’s mission, vision, and values, adhering to all policies and procedures.
• Participate in required educational programs relevant to the position.
• Perform other related duties as assigned.

Qualifications

Education

• Required: Graduate of an accredited occupational therapy program recognized by the American Medical Association and the American Occupational Therapy Association.
• Desired: Continuing education relevant to practice area and skill level.

Licenses/Certifications/Registrations

• Required: Licensed as an Occupational Therapist in the State of Oregon.
• Valid driver's license and proof of auto insurance.
• Current certification in Basic Life Support (BLS).
• Preferred: Registration as an Occupational Therapist with the American Occupational Therapy Association.

CRASE training will be required if the travel assignment extends beyond six months.
