Job summary

Healthforce seeks an Occupational Therapist for a 13-week travel assignment in Keene, New Hampshire. The position offers 32 hours per week with a 5x8 shift (08:00–16:30) in a Skilled Nursing Facility setting. Start date is 11/23/2026, with weekly pay in the range of $1,823–$1,915.

This is a travel - temporary assignment designed for licensed OT professionals. The role emphasizes collaboration with an interdisciplinary team, ongoing assessment, documentation, and discharge planning to support

Qualifications

• Active OT license in the United States as applicable to the state of practice.
• Master’s or Doctorate in Occupational Therapy (OT) as required.
• Experience or demonstrated ability to work in a skilled nursing facility or rehabilitation setting.
• Willingness to travel for a 13-week assignment and to work a 32-hour minimum weekly schedule.
• Strong communication and documentation skills; ability to work within a multidisciplinary team.

Responsibilities

• Evaluate residents’ functional status and develop individualized occupational therapy treatment plans.
• Implement therapeutic interventions to improve ADLs, mobility, balance, strength, and endurance.
• Reassess progress, document outcomes, and update goals; maintain accurate records per standards.
• Collaborate with an interdisciplinary team to coordinate care with nursing, therapy colleagues, social work, and aides.
• Contribute to discharge planning and transitions of care for residents returning home or to other settings.
• Ensure safety, infection control, and regulatory compliance during therapy sessions.
