Responsibilities
- Perform plan of care intervention including medication administration, specimen collection, clinical treatments, and other medical care, documenting care given.
- Monitor patient condition and communicate with care team, physician, patient, and family as appropriate.
- Coordinate patient care across disciplines, including patient transfer, discharge, referral, and psychosocial support needs.
- Evaluate learning needs of patient and/or family and provide education appropriate to age, culture, condition, and circumstances.
- Act as advocate for the physical and emotional well-being of the patient.
