TNAA TotalMed RN is seeking a travel nurse RN Long Term Acute Care for a travel nursing job in Wilmington, Delaware.

Job Description & Requirements

We're looking for Registered Nurses (RNs) for an immediate travel opening in an LTAC (Long-Term Acute Care) facility in Wilmington, DE. The right RN should have at least 1 year of recent experience in a Long-Term Acute Care or similar high acuity setting.

Responsibilities

As an RN in LTAC, you will care for patients with serious medical conditions who require extended hospital-level care. These patients often have complex needs such as ventilator support, wound care, or post-ICU recovery. You will be responsible for providing direct nursing care, monitoring patients' conditions, and collaborating with an interdisciplinary team to support patient recovery and discharge planning.

• Perform head-to-toe assessments and respond promptly to changes in patient condition
• Administer IV medications, manage central lines, and monitor vital signs and lab results
• Provide ventilator and tracheostomy care, as well as advanced wound management
• Collaborate with respiratory therapists, physicians, and case managers for ongoing treatment plans
• Support patients with mobility, hygiene, and nutritional needs
• Supervise LPNs and CNAs, ensuring proper patient care delivery
• Adhere to strict infection control protocols and facility safety standards
• Participate in interdisciplinary rounds and communicate patient status updatesDocument all care activities and interventions accurately and in real time
• Assist with admissions, transfers, and discharge planning

The ideal candidate must have at least one year of experience as an RN in a Long-Term Acute Care setting. Additionally, they should possess a current BLS certification and be eligible for State licensure. Additional certifications may be required.
