Qualifications

• Current, unrestricted RN license in the state of practice.
• Minimum of 1 year nursing experience in a clinical care setting.
• Valid driver’s license and reliable transportation.
• Willingness to travel within assigned service area (up to 50%).
• ASN or BSN in Nursing preferred.
• Prior Home Health or Hospice experience preferred.
• Familiarity with EMR systems (e.g., HomeCare HomeBase) preferred.

Responsibilities

• Perform initial and ongoing assessments of patients’ physical, psychological, social, and environmental needs.
• Develop and implement individualized care plans with patients, families, and interdisciplinary team.
• Reassess patients regularly and adjust care plans as condition changes.
• Communicate significant findings and changes to supervisors and care team.
• Maintain accurate clinical documentation per regulatory standards.
• Uphold patient confidentiality and privacy (HIPAA).
• Collaborate with clinicians to ensure coordinated, high-quality care.
