Job summary

VyTalent Solutions is seeking a Registered Nurse for a Home Health travel assignment in Cordova, TN. This 13-week contract offers an 8-hour daily schedule with on-call rotations and a competitive pay range.

The RN will assess and manage care for well, chronic, or acutely ill individuals in a community setting, coordinating with other health professionals and ensuring safe medication practices. OASIS and HCHB experience are preferred.

Qualifications

• Active RN license (compact license accepted).
• BLS certification required.
• Experience with home health or community-based care preferred.

Responsibilities

• Assess, plan, implement, and evaluate nursing care for homebound patients.
• Collaborate with multi-disciplinary teams to address health needs.

Pay Range: 1880.77 - 1974.81

Schedule: 8am-4:30pm EST (On Call required every 3 weeks at rate of $25 per day and $50 per weekend day/holiday)

Contract Length: 13 weeks

Requirements

Registered Nurse License willing to accept compact license as well. BLS certification, T step TB test. OASIS, HCHB, Wound Competency preferred but Wound class is offered if needed.

Responsibilities

• Assess, plan, implement, and evaluate nursing support for well, chronic, or acutely ill individuals in a community or small facility setting.


• Collaborate with other health professionals to plan and evaluate programs addressing the health needs of persons served.


• Coordinate communication and actions among disciplines related to health and safety supports.


• Ensure medication policies are in place, reviewed annually, and adhered to, including ordering, monitoring, and disposal of medications.


• Complete and maintain all required nursing documentation and reports, ensuring timely responses to changes in patient conditions.
