GuideStar Eldercare seeks a Nurse Practitioner to travel daily to nursing homes and assisted living facilities, delivering initial evaluations, ongoing therapy, and cognitive testing as part of a private-practice style role with regional leadership support.

You will document services, collaborate with an interdisciplinary team, and build strong relationships with facilities, patients, and families while upholding high standards of safety and dignity.

Qualifications

• Must hold valid Nurse Practitioner licensure to practice.
• Able to work as part of an interdisciplinary treatment team.
• Strong time management skills.
• Ability to function autonomously with regional Area Manager support.
• Must pass pre-employment background check and drug screening.
• Must complete credentialing process.

Responsibilities

• Travel daily to assigned facilities from home.
• Provide initial evaluations, ongoing therapy services, and cognitive testing.
• Document the need and extent of services for each patient.
• Develop and maintain positive relationships with client facilities, patients, and families.
