Job summary

Northshore Home Health Care is seeking an RN for Med Surg / Telemetry in Georgia. You will assess patient status, coordinate care with physicians and the clinical team, and support safe transitions of care, including handoffs and updates to care plans.

The role requires a minimum of 2+ years of nursing experience, strong assessment and coordination skills, and the ability to communicate effectively with the team.
