Job summary

Healthforce is seeking a Travel Long-Term Care (LTC) LPN to join our team in Ossian, IA. You will provide high-quality nursing care in a skilled nursing facility, manage chronic conditions, and perform routine assessments for residents in a collaborative, patient-centered environment.

The role requires LPN licensure in Iowa (or a valid Compact license), CPR/BLS, and at least 1 year of LTC/SNF experience. Expect rotating 12-hour shifts, every other weekend and holiday, with competitive weekly pay

Qualifications

• Active LPN license in Iowa or compact license.
• CPR/BLS certification.
• Minimum 1+ year LTC/SNF experience.
• Willingness to rotate day/night shifts and weekends/holidays.

Responsibilities

• Monitor vital signs, administer medications, document responses.
• Perform wound care and routine procedures per care plans.
• Communicate changes to RNs, physicians, and families promptly.
• Assist with ADLs and rehabilitation goals as directed.
