Job summary

Trilogy Health Services, LLC is seeking a Travel Licensed Practical Nurse to support short- and long-term assignments across Indiana, Ohio, Michigan, and Kentucky. The role involves providing direct, quality clinical care and supervising nursing assistants on the unit.

Responsibilities

• Visit residents daily to assess physical and emotional status and notify charge nurse or physician of changes.
• Identify problems and emergencies and initiate lifesaving measures when physician is unavailable.
• Provide holistic LPN-directed care within license scope.
• Monitor medication inventory and supplies to ensure availability for residents.
• Complete medical forms, reports, and charting as required.
• Report exposures to blood, body fluids, and hazardous materials per policy.

POSITION OVERVIEW

This Travel Licensed Practical Nurse role supports short- and long-term assignments across Indiana, Ohio, Michigan, and Kentucky, requiring adaptability while maintaining consistent standards of care. The Licensed Practical Nurse is responsible for providing direct, quality clinical care and the supervision of nursing assistants working with residents assigned to each unit.

Key Responsibilities

• Visit residents on assigned unit daily to observe and evaluate each resident’s physical and emotional status and notify the charge nurse, the physician, and/or the Director of Nursing when there is a change in a resident’s condition.
• Identify resident problems and emergency situations and initiate immediate lifesaving measures in the absence of a physician.
• Provide holistic care for residents within the scope of your LPN license.
• Assist in monitoring the inventory of medications, medical supplies and equipment to ensure that an adequate supply is on hand and readily available to meet the needs of the residents.
• Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc. as necessary
• Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the facility’s policies and procedures governing accidents and incidents.

Qualifications
