Qualifications

Responsibilities

• Provide direct patient care to residents on the post-acute unit under RN/charge nurse supervision.
• Administer prescribed medications and treatments per physician orders; monitor and document patient responses.
• Perform or assist with phlebotomy, EKGs, and transports as permitted; manage IV pumps with appropriate certification.
• Assist residents with activities of daily living and participate in restorative/rehabilitative activities.
• Monitor for changes in condition and report to the care team.
• Maintain accurate documentation in the EPIC EMR and communicate changes to the care team.
