Job summary

Qualifications

• Master’s degree in Social Work from CSWE-accredited program required.
• Active LCSW license in the state of practice required.
• Minimum of one year of social work experience preferred.
• Current CPR if applicable.
• TB questionnaire/PPD or chest x-ray if applicable.
• Current health certificate per contract or state regulation.
• Must meet all federal, state and local requirements.
• Must be at least 18 years of age.

Responsibilities

• Collaborates with the multidisciplinary team to develop the plan of care for patients/clients needing social work intervention.
• Provides counseling and crisis intervention to patients/clients with social or emotional problems across settings and age ranges.
