Qualifications

• Graduate of school of pharmacy (Doctor of Pharmacy degree preferred).
• Pharmacist license in the state of practice (required).
• Licensures and certifications needed: ACLS, BLS, PALS (required).
• Minimum inpatient pharmacy experience: 1 year preferred; 1 year technical experience preferred.

Responsibilities

• Verify and fill medication orders, monitor patient therapies, provide drug information.
• Identify and troubleshoot potential medication safety hazards; document interactions for medical review.
• Supervise support personnel; collaborate with healthcare teams to optimize medication management.
• Participate in medication management activities and quality assurance initiatives.
