Qualifications

• Active RN license in CT or compact license.
• Minimum 1 year of nursing experience, preferably in hospice or home health.
• Nursing duties include direct patient care, assessment and coordination of hospice services.

Responsibilities

• Provide direct patient care to hospice patients during night shifts.
• Assess patient needs and adjust care plans in collaboration with physicians.
• Document care and charting using Oasis and Homecare Homebase systems.
