Fusion Medical Staffing seeks a Travel Home Health RN for a 15-week assignment in Kingston, PA. The RN will deliver direct home health care, assess patients, and coordinate with the clinical team to ensure quality care in patients' homes.

Requirements include 1 year RN experience with at least 6 months in Home Health, proficiency with OASIS, valid RN license, and BLS certification. Travel-friendly compensation and comprehensive benefits are provided.

Formación

• Requires 1 year RN experience with at least 6 months in Home Health
• Proficient in OASIS documentation
• Current RN license in good standing; BLS required

Responsabilidades

• Assess patient conditions and develop individualized care plans
• Administer meds, treatments, and wound care; monitor responses
• Educate patients and caregivers on meds, disease, and self-care
• Monitor changes in patient status and adjust care plans as needed
• Coordinate with physicians, therapists and other healthcare professionals
• Document care, interventions and progress per regulatory standards
• Ensure infection control and safety in home care settings
• Support families and caregivers with guidance and emotional support
• Maintain communication with case managers and interdisciplinary teams
• Complete charting and documentation; meet visit/productivity goals
• Perform other duties within scope of practice
