Qualifications

• One year of recent RN experience, with at least 6 months in Home Health.
• Proficient in OASIS documentation.
• Valid RN license per state regulations and BLS certification.

Responsibilities

• Assess patient conditions and develop individualized care plans.
• Administer medications, treatments, and wound care with monitoring.
• Educate patients and caregivers on disease management and self-care.
• Coordinate with physicians and therapists for seamless care transitions.
• Document care and progress in compliance with standards.
