Supplemental Health Care is seeking a travel Home Health Physical Therapist for a 13-week assignment in Sumter, South Carolina. The role involves 40 hours per week, with 8-hour day shifts and travel between patient homes.

Qualifications include an active PT license, current BLS, 1+ year home health experience, and proficiency with EMRs (Homecare Homebase, NetSmart, Epic). Travel-ready with reliable transportation is required.

Qualificações

• Active PT license in the state of assignment.
• Current BLS certification.
• At least 1 year of home health experience.
• Experience conducting PT evaluations and developing therapy plans.
• Familiar with Medicare-certified home health guidelines and OASIS-E.
• Experience with EMRs such as Homecare Homebase, NetSmart, or Epic.
• Clear background check, driving record, and up-to-date health clearance (TB, immunizations).
• Reliable transportation with valid auto insurance and ability to travel between patient homes.

Responsabilidades

• Perform initial and ongoing PT assessments and develop individualized plans of care.
• Provide skilled interventions for strengthening, endurance, pain management, and fall prevention.
• Conduct gait and balance evaluations, home safety assessments, and mobility training.
• Educate patients and caregivers on exercise programs, adaptive techniques, and safety strategies.
• Collaborate with RNs, OTs, SLPs, and physicians to ensure coordinated, interdisciplinary care.
• Document all care in compliance with Medicare, OASIS-E, and agency standards.
