Job summary

Supplemental Health Care is seeking a travel Home Health Physical Therapist for a travel assignment in Aiken, SC. The role is 40 hours/week, 8-hour days, for 13 weeks, starting ASAP. Travel between patient homes is required and EMR systems such as Homecare Homebase, NetSmart, or Epic are used.

The successful candidate will perform assessments, develop plans of care, and collaborate with the care team. Compensation is paid weekly and includes full benefits.

Qualifications

• Active, unrestricted Physical Therapist license in the state of assignment.
• Current BLS certification (AHA).
• 1+ year of recent home health experience.
• Experience conducting PT evaluations and developing therapy plans.
• Familiar with Medicare home health guidelines and OASIS-E documentation.
• Experience with EMRs such as Homecare Homebase, NetSmart, or Epic.
• Clear background check and health clearance, and ability to travel between patient homes.

Responsibilities

• Perform initial and ongoing PT assessments and develop individualized plans of care.
• Provide skilled interventions for strengthening, endurance, pain management, and fall prevention.
• Conduct gait and balance evaluations, home safety assessments, and mobility training.
• Educate patients and caregivers on exercise programs and safety strategies.
• Collaborate with RNs, OTs, SLPs, and physicians for coordinated care.
• Document all care in compliance with Medicare, OASIS-E, and agency standards.
