Supplemental Health Care is seeking a travel Home Health Physical Therapist to support patients in Scottsboro, Alabama. This 13-week assignment offers 40 hours per week, with eight-hour day shifts, and a focus on patient-centered home health care.

Applicants must hold an active PT license in the state, current BLS (AHA), and at least 1 year of recent home health experience. EMR experience and reliable transportation are required to travel between patient homes.

Qualifications

• Active, unrestricted PT license in the assignment state.
• Current BLS certification (AHA).
• 1+ year of recent home health experience.
• Experience conducting PT evaluations and developing therapy plans.
• Familiar with Medicare-certified home health guidelines and OASIS-E.
• EMR experience with Homecare Homebase, NetSmart or Epic.

Responsibilities

• Perform initial and ongoing PT assessments and develop plans of care.
• Provide interventions for strengthening, endurance, pain management, and fall prevention.
• Conduct gait and balance evaluations and home safety assessments.
• Educate patients and caregivers on exercises and safety strategies.
• Collaborate with RNs, OTs, SLPs, and physicians for coordinated care.
• Document care according to Medicare, OASIS-E, and agency standards.
