Ardor Health Solutions is seeking a Travel Physical Therapist for a Home Health contract in Upper Marlboro, Maryland. This assignment offers autonomy and one-on-one care in patients' homes, with a 36-hour weekly guarantee and a Monday–Friday schedule from 8:00 AM to 5:00 PM.

The ideal candidate enjoys autonomous, patient-centered care and has Maryland PT licensure with BLS; Home Health experience is preferred. Counties covered include Prince George’s and Montgomery.

• Perform comprehensive physical therapy evaluations and reassessments in patient homes
• Develop and implement individualized home health treatment plans

• Provide gait, balance training, therapeutic exercise, transfer training and fall prevention education
• Educate patients and caregivers on home exercise programs and safety
• Coordinate with nurses, physicians, case managers and care teams
• Complete timely documentation and maintain home health compliance
• Manage home health caseload and plan visits based on patient needs
