Job summary
Host Healthcare is seeking a travel Home Health Physical Therapist for a 13-week assignment in Lacey, WA. The role requires 40 hours per week with 8-hour daytime shifts. Start date is 11/23/2026. Qualifications
- Licensed Physical Therapist with current state licensure.
- Experience in home health or pediatric/geriatric therapy is a plus.
- Ability to work independently in patients' homes and document sessions.
Responsibilities
- Provide comprehensive PT services in home health settings.
- Develop and adjust treatment plans based on patient needs.
- Perform initial evaluations and discharge planning; coordinate with care teams.
