American Traveler is seeking a Physical Therapist for a Home Health position requiring 1+ year of home health experience and OASIS proficiency.

• Documentation skills including OASIS, plan of care, evaluations, and long/short term goals
• Experience with cardiopulmonary, geriatric, orthopedic, and neurologic patient populations preferred
• Experience with wound care, vestibular rehab, and lymphedema management preferred
• Familiarity with home health equipment including gait belts, walkers, hospital beds, and lift devices
• Knowledge of home exercise programs, patient/family education, and safety/injury prevention

Additional Information

• Responsibilities include evaluating and treating patients in their home environment across a variety of diagnoses and conditions
• Documentation duties include OASIS assessments, care plans, and progress notes
• Candidates may occasionally float to other facilities within the health system as operationally necessary
• Two professional references from the past 2 years required, with at least one from a supervisor
• Full 7-year work history with any gaps explained is required for consideration
• All time off requests must be disclosed and approved prior to contract signing
• Local candidates are not accepted; must reside more than 60 miles from the facility
• Previous PeaceHealth employees must have been separated from the health system for at least 1 year before returning as a traveler
• Candidates with more than one DUI are not eligible for consideration
• First-time travelers are accepted if they have OASIS experience

American Traveler Job ID #P-778544. Pay package is based on 8 hour shifts and 40.0 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
