Supplemental Health Care is seeking a travel Home Health Physical Therapist for Brewton, Alabama. This 13-week assignment offers 40 hours weekly at 8-hour shifts, with travel between patient homes as part of the job.

You will assess and treat patients in their homes, develop plans of care, and collaborate with RNs, OTs, SLPs, and physicians. A current Alabama PT license, BLS, and 1+ year home health experience are required.

Qualifications

• Active, unrestricted PT license in Alabama
• Current BLS certification (AHA)
• 1+ year of recent home health experience
• Experience conducting PT evaluations and developing therapy plans
• Familiar with Medicare-certified home health guidelines and OASIS-E documentation
• Experience with EMRs such as Homecare Homebase, NetSmart, or Epic
• Clear background check, driving record, and up-to-date health clearance (TB, immunizations)
• Reliable transportation with valid auto insurance and ability to travel between patient homes

Responsibilities

• Perform initial and ongoing PT assessments and develop individualized plans of care
• Provide skilled interventions for strengthening, endurance, pain management, and fall prevention
• Conduct gait and balance evaluations, home safety assessments, and mobility training
• Educate patients and caregivers on exercise programs, adaptive techniques, and safety strategies
• Collaborate with RNs, OTs, SLPs, and physicians to ensure coordinated, interdisciplinary care
• Document all care in compliance with Medicare, OASIS-E, and agency standards

• Experience conducting PT evaluations and developing therapy plans

Home Health Physical Therapist Contract Details:

• $2,138 - $2,309 per week*
• 13-week contract
• Paid by the hour - not by the visit
• 5-6 visits per day (45-90 minutes each, based on complexity)
• Travel time factored into productivity expectations
